Prerequisites and basic notions

Before facing the "Practical-evaluative internship for the state exam - medical area," the student must have passed the internship of "DIDACTIC PRACTICE OF MEDICAL SEMIOTICS" and those of "DIDACTIC PRACTICE OF HEMATOLOGY, GASTROENTEROLOGY, INFECTIOUS DISEASES, RHEUMATOLOGY, DERMATOLOGY, AND TUTORIAL PROBLEM-BASED LEARNING. " The student should therefore have already acquired knowledge and skills in the medical field that allow him to correctly collect the medical history, execution of the complete physical exam, and the choice of diagnostic tests consistent with the hypothesized diagnosis.

Program

The program covers all internist and geriatric diseases that may be encountered in patients admitted to a geriatrics or internal medicine ward. For a detailed syllabus refer to the syllabi of the teachings of clinical pathophysiology, medical semeiotics, internal medicine and clinical medicine.

Didactic methods

Students are asked to follow their assigned patients at every stage of hospitalization: admission from the Emergency Department or the territory, inpatient stay, completion of diagnostic investigations, setting of therapy, discharge, and follow-up planning, working alongside and assisting the medical mentor. At the same time, they will have to review individually, using internal medicine and medical therapy texts and/or the most accredited and up-to-date guidelines the principles of pathophysiology, differential diagnosis, and therapy of the diseases from which patients admitted to the ward are affected. Thus, the learning process will have to take place both practically, by performing independently (albeit in a supervised manner) some basic medical acts (collecting medical history, performing objective examination), actively participating in planning diagnostic examinations, setting therapy and follow-up, and theoretically, by demonstrating the ability to search bibliographic sources for information useful in understanding the patient's problems.

Learning assessment procedures

There is no formal examination. the student will be judged by the tutor throughout the internship.

Evaluation criteria

The assessment will explore the following components of the medical profession (see last page of the booklet):
The student…
- Implements good practices of the physician-patient relationship (interview, relationship, information, clarity, obtaining consent, respect for privacy)
- Has the ability to collect history and perform an objective examination in an outpatient and/or hospital setting
- Is able to correctly report the history and objective examination in the medical record
- Knows and can apply clinical reasoning, has the ability to identify priority or urgent and secondary problems, has the ability to propose diagnostic hypotheses and identify diagnostic tests of higher sensitivity and specificity to confirm or not confirm these hypotheses
- Is able to interpret laboratory tests
- Is able to interpret diagnostic imaging test reports
- Is oriented on decision-making processes related to pharmacological and non-pharmacological treatment. Is able to propose therapy appropriate to the pathology and the specific patient
- Is able to fill out the admission/dismissal report for hospitalization; is able to fill out the discharge letter
- Is able to assess the appropriateness of the indication for hospitalization and indicate paths to rehabilitation or sheltered hospitalization in other facilities
- Demonstrates the ability to frame the reason for hospitalization within the complex of any chronicities, other criticalities, and frailties of patients
- Can recognize his/her own cognitive limitations by demonstrating the ability to fill gaps through consultation of appropriate texts and guidelines
- Can succinctly but comprehensively present clinical cases to department colleagues or outside consultants interacting pro-actively
- Knows how to indicate preventive and health education actions
- Demonstrates knowledge and awareness about the organization of the National Health Service and the Regional Health Service
- Respects shift start and end times, dresses appropriately for the role, carries all necessary equipment. Observes hygiene rules, washes hands between patient visits
- Interacts properly with medical, nursing and technical staff on the ward
- Demonstrates knowledge and awareness of ward (or outpatient) rules
- Demonstrates knowledge and awareness of the different roles and duties of team members
- Demonstrates an active attitude (asks questions, proposes to carry out activities)

Criteria for the composition of the final grade

For a POSITIVE EVALUATION of the above components of the medical profession, a synthetic letter score will be used with values corresponding to:
A: Excellent;
B: Excellent;
C: Good;
D: Satisfactory;
E: Sufficient.
For a NEGATIVE rating, the letter will be used.
F: Insufficient
Taking all these aspects into account, the student will be judged as either "qualified" or "not qualified".
