Learning outcomes

Educational Objectives
This course equips students with the knowledge and diagnostic skills required to make meaningful contributions to the discipline of cardiology in physiotherapy and cardiological rehabilitation.
The course aims to enable the students:
- to know the anatomical, etiological and pathophysiological mechanisms of the major cardiovascular diseases, and particularly of coronary syndromes, acute myocardial infarction, and heart failure;
- to look into preventive management of cardiovascular risk factors including hypertension, dyslipidemia, diabetes mellitus, obesity, and smoking and into public programs to treat cardiac emergencies.
- to know the pathophysiological bases and mechanisms of cardio-vascular system, coronary vessels, and arterial blood pressure behavior during physical effort;
- to know major symptoms and key signs of the main heart diseases of interest for cardiac rehabilitation; to learn a brief outline of the main diagnostic methods and therapies;
- to achieve specific skills and knowledge for:
- a general approach to heart patient in the field of cardiac rehabilitation
- the distinction of the different stages of a rehabilitation program and the purposes of rehab training;
- rational planning of physical exercise and training in the heart patient;
- the knowledge of the pathophysiological bases for moderate-intensity continuous training and high-intensity interval training in cardiac rehabilitation settings;
- the principles of effective and safe prescription of exercise intensity and training modalities based on the results of either ergometric or cardiopulmonary (spiroergometric) testing on a bicycle or treadmill;

Program

Program of cardiac rehabilitation
1. Brief outline of Anatomy and Physiology of Heart: cardiac pump, coronary vessels, valves, and electrical conduction system of the heart.
2. Brief outline of the Pathophysiology, main symptoms and signs, and therapies of the main heart diseases of interest for cardiac rehabilitation: 1) myocardial diseases, 2) valve diseases, 3) coronary syndromes, 4) heart failure.
3. Main symptoms and signs of heart disease with brief remarks on cardiovascular diagnostic methods.
4. Effects of physical exercise on patients with heart failure: central and peripheral mechanisms of exercise intolerance; indications to treatment and prescription.
5. Stages of cardiac rehabilitation (and historical development).
6. The professionals involved in cardiac rehabilitation including cardiologists and physiotherapists.
7. Clinical and instrumental evaluation of heart disease: types of disease, ejection fraction on echocardiography, ischemic threshold; VO2 peak and severity grade.
8. Assessment of functional capacity: criteria, modalities; ergometric testing; cardiopulmonary exercise testing, rating of perceived exertion (so-called Borg scale).
9. Cardiac rehabilitation training:
 Moderate-intensity continuous training and high-intensity interval training;
 Exercise intensity: percent of VO2 peak; ventilatory thresholds, percent of heart rate peak, percent of heart rate reserve, Borg scale.
 The basis for the prescription of rational planning of physical exercise and training in the heart patient;
 Examples of typical training sessions.
 Monitoring of patient during exercise training
 Outline of electrocardiographic monitoring during exercise training and tests.
 Beneficial effects and outcome of exercise Training during follow-up.
 Home cardiac rehabilitation
10. Prevention of coronary heart diseases and risk factors.
11. Types of heart patients affected by coronary syndromes, heart failure, previous heart surgery.

Examination Methods

Teaching Methods
The lessons are theoretical and will include oral lessons and distance learning, lasting 10 hours in total. The lectures will be supported by projection of PowerPoint presentations that will be provided to students in pdf format. PDF handouts will be distributed or sent by email at the end of the lessons or of the course.
