Principles of taking care of the person with chronic degenerative disease:
- promote patient autonomy and independence and reduce the risk of disability
- promote the self-management of the disease
- guarantee continuity of care: transitions, discharge, treatment reconciliation
- understand the disease trajectory
Effects of hospitalization in chronic conditions and strategies to avoid the "hospitalization syndrome"
Problems of the elderly person and / or with chronic degenerative diseases:
Fall risk: risk factors, rating scales and prevention measures
Cognitive state and cognitive disorders: evaluation and management of memory deficit, language deficit, perception deficit
Prevention of delirium: assessment and surveillance interventions (use of appropriate rating scales)
Communication with the elderly without or with cognitive impairment: ageism and elderspeak
Stroke
Assessment and protocol in the first 24 hours
Assessment and rehabilitation-educational approach in the post-acute phase with particular attention to
early mobilization, to outcomes related to disability and family needs
Diabetes mellitus
The educational triad: drug / insulin treatment, diet and nutrition and physical activity
- Self-care behaviors of acute complications: hypoglycemia and hyperglycemia

Didactic methods
Interactive lessons will be used, students will be actively encouraged to integrate theory with previous practical experiences and/or integrate – link new knowledge with those covered in other modules.
Learning assessment procedures
Test exam with closed questions with multiple choice answer and / or open questions with short answer, semi-structured questions (ex. paragraph completion, construction of a therapeutic scheme ...).
Test may include an oral exam to supplement the written test.
