Learning outcomes
The course aims to provide the contents and methods for:
- assist the person with chronic medical problems in the phase of stability and instability (heart failure, IMA and angina, COPD and asthma).
- promote specific self-care behaviors in the patient, considering that most of the patients with chronic illness live at home and the hospitalization time is very short.
The problems of the person will be addressed considering their evolution, the evaluation of the patient reasoned and the choice of care interventions based on evidence, appropriateness and needs of the patient. The instability / exacerbation will be addressed with a welfare protocol. The impact and the experience of the disease on the life of the patient and on the family will be considered and the aspects of rehabilitation and palliative will be explored with respect to the symptoms in the advanced stage (eg dyspnoea).
This teaching is built on the knowledge of the 1st year of "General Nursing and Applied Methodology" and "Fisiopatology applied to nursing" (assessment of breath, meaning and signs / symptoms of hypoxia, dyspnea, principles of O2 therapy, principles of caring and supervision). The contents are linked to the modules of "Therapeutic Education" and "Community Nursing" for the trend of early discharge and need for support at the time of discharge.
The teaching also integrates with the professional laboratories scheduled for the 2nd year of the course (Making decisions for the management of the patient with heart failure in the stabilized phase; Developing self-care skills in a person with COPD).
Module objectives:
Analyze and understand the problems of care and the areas of vigilance connected to the main chronic degenerative diseases, in the different phases of trajectory (acuity, stability, chronicity). Describe and analyze interventions aimed at managing these problems and developing self-care skills with a view to continuity in the different care settings.
