-Descrivere i meccanismi patogenetici fondamentali di malattie e sindromi rilevanti.
-In esse correlare alterazioni cellulari, alterazioni di funzione d’organo e di apparato, adattamenti legati ai meccanismi omeostatici, riconoscendone le manifestazioni cliniche fondamentali.
Program
• Fabbisogni fondamentali delle cellule: apporto di O2, apporto di nutrienti ed eliminazione di metaboliti, mantenimento dell’equilibrio idro-elettrolitico ed acido-base.
Alterazione dell’apporto di ossigeno (anemie: in particolare anemie carenziali, iporigenerative ed emoglobinopatie).
• Equilibrio idroelettrolitico dell’organismo: mantenimento dei volumi e distribuzione dei liquidi e loro alterazioni , flussi intercompartimentali dei fluidi, sindromi da iperidratazione e disidratazione, equilibrio intra-extracellulare dei fluidi (soluzioni isotoniche, ipertoniche ed ipotoniche), composizione dei fluidi e sistemi tampone intra ed extracellulari, mantenimento dell’equilibrio idroelettrolitico (funzione renale, ADH, aldosterone, ANP) e sindromi edemigene, alterazioni dell’equilibrio acido-base e attivazione dei meccanismi di compenso (acidosi ed alcalosi metabolica e respiratoria). Cause e conseguenze delle alterazioni dei principali elettroliti (Na+, K+, Cl-, Ca++)
• Apparato endocrino: principali ormoni con attività metabolica e di regolazione del circolo e dei volumi, iper ed ipofunzione delle principali ghiandole endocrine( ipofisi, surrene, tiroide, paratiroidi), risposta allo stress (catecolamine, asse ipotalamo-ipofisi-surrene), diabete mellito, ipoglicemia.
• Apparato respiratorio: alterazione della ventilazione, della diffusione alveolo-capillare dei gas, del rapporto ventilazione-perfusione alveolare. Insufficienza respiratoria..
• Apparato cardiocircolatorio: controllo del tono vascolare e della pressione arteriosa, ipertensione arteriosa, cenni alle principali patologie cardiache , scompenso cardiaco , shock.
• Trombosi ed embolie arteriose e venose. Embolia polmonare.
• Apparato urinario: cenni alle malattie renali, insufficienza renale acuta e cronica.
• Apparato digerente: malnutrizione, maldigestione, malassorbimento; cirrosi epatica, ipertensione portale, ascite.
• Sofferenza del sistema nervoso centrale a seguito di alterazioni metaboliche (ipossia, ipercapnia, ipoglicemia, ipertonia ed ipotonia extracellulare)
