Learning outcomes
The course aims to encourage students to develop an approach to clinical and care management of people affected by disease with an ongoing chronic that accompany them during their daily life and for their entire existence. Physiopathological, clinical, welfare and educational aspects of some priority health problems selected on the basis of their epidemiological relevance and exemplariness, both in the situation of clinical instability but above all in the
chronicity / disability. The impact and experience of the disease on the patient's life and family will be considered and the aspects of rehabilitation and palliation of symptoms in the advanced stage.
Specifically, the course aims to:
- recognize the problems in the different phases of the evolutionary dynamics of the disease: stability, exacerbation and advanced phase
- develop assistance and educational skills towards the person and family in the phases of stability-instability, in the transition processes, and in redefining the
lifestyle, adapt to continuous therapeutic regimens, prevent complications
- consider the patient and family as competent partners in the care plan;
- promote a palliative approach in the management of symptoms and pain during all stages of the disease and at the end of life
This course is built on the knowledge of the 1st year of "General Nursing and Applied Methodology" and "Applied Physiopathology
to nursing "(principles of caring: centrality of the family, supporting self-care, empowerment; mobility and dependence deficit in ADLs; assessment of nutritional status; pain). The contents are linked to the modules of "Therapeutic Education" and "Community Nursing" for the trend of early discharge and need for support at the time of discharge; to the module of "Anesthesiology and analgesic therapy" (Physiology and pathophysiology of pain transmission).
The teaching also integrates with the professional workshops planned for the 2nd year of the course (Developing self-care skills in a person with diabetes; Making decisions for the management of patients with acute and chronic liver cirrhosis).
Program
Principles of taking care of the person with chronic degenerative disease:
- promote patient autonomy and independence and reduce the risk of disability
- promote the self-management of the disease
- guarantee continuity of care: transitions, discharge, treatment reconciliation
- understand the disease trajectory
Effects of hospitalization in chronic conditions and strategies to avoid the "hospitalization syndrome"
Problems of the elderly person and / or with chronic degenerative diseases:
Fall risk: risk factors, rating scales and prevention measures
Cognitive state and cognitive disorders: evaluation and management of memory deficit, language deficit, perception deficit
Prevention of delirium: assessment and surveillance interventions (use of appropriate rating scales)
Communication with the elderly without or with cognitive impairment: ageism and elderspeak
Stroke
Assessment and protocol in the first 24 hours
Assessment and rehabilitation-educational approach in the post-acute phase with particular attention to
early mobilization, to outcomes related to disability and family needs
Diabetes mellitus
The educational triad: drug / insulin treatment, diet and nutrition and physical activity
- Self-care behaviors of acute complications: hypoglycemia and hyperglycemia
- Acute situations requiring integrated intervention
- Interventions for the prevention of chronic complications
Parkinson's and Alzheimer's
- Assessment, definition and expressions of the main clinical manifestations;
- Problems and clusters of symptoms during the disease trajectory from the pre-clinical stage to the advanced stage of the disease
