Learning objectives

The goals are to provide basic knowledge about biomedicine, hygienic prevention and fundamental principles of professional discipline in order to better approach the first internship experience. The ultimate purpose is for the student to acquire the basic skills so as to orientate them in the management of extracorporeal circulation and in the blood management.

Prerequisites and basic notions

Knowledge of anatomy, physiology, biomedical and basic preventive hygiene knowledge

Program

Technical Skills:
a)Before patient entry Cardiac Surgery operating room:
- orientation in the operating block and recognition of rooms of interest (Coordinators' guardhouse, operating rooms, 10-13 storage for materials restoration, materials storage for equipment sanitization)
- acquisition patient info operating list, type of surgery, Tutor room assignment.
- BSA pz calculation, nominal theoretical flow with related choice of materials to set up
- Equipment arrangement in the s.o.
- Setup of intraoperative hemo-recovery machine
(b) At patient arrival in the room:
- Heat exchanger thermal mattress circuit connection
- Patient's request for confirmation of weight and height
- Consultation of patient's medical record
- Oxy placement and hydrodynamic testing
- Orderly and safe arrangement of heat exchanger tubes
- Setting up the extracorporeal circuit with understanding of circuit logic and maintaining sterility
- Performing the pre-CEC checklist along with the Tutor
- selection of cannulas for ECC together with Tutor
- identification of major anatomical structures during open heart surgery.
(c) During surgery:
- Autonomous management of cell saver
- Autonomous management of electronic chart and understanding of values
- Interpretation of blood gas values and technical characteristics of ECC conduction
- Autonomous management of heat exchanger unit
(d) Post CEC:
- restoration of consumed materials
- Disposable circuit removal and equipment sanitization after patient chest closure
Non-Technical Skills:
- observance of behavioral norms to be followed in the operating group (uniform, headset, no jewelry, use of cell phone in s.o.,etc.)- confidentiality of information acquired during the internship (sensitive data pcs etc..)- effective communication with Tutor regarding activities and respect for roles within the s.o.
- understanding of the stages of the surgery and curiosity in delving into technical and/or surgical steps with the relevant Tutor.
- continuity and consistency in attending the proportionate internship during the academic year

Evaluation of stage period with final theoretical-practical examination

Students with disabilities or specific learning disorders (SLD), who intend to request the adaptation of the exam, must follow the instructions given HERE

Evaluation criteria

1) Insufficient: the student does not attend internship at all or attends little by not reaching the minimum amount of hours stipulated in the educational regulations not showing interest in internship activities;
the student attends internship reaching the minimum amount of hours not showing interest in the activities without reaching the minimum objectives set.

2) Sufficient: the student attends internship with low interest in the discipline reaching the minimum amount of hours established by the didactic regulations. Performs technical activities only on the Tutor's instructions.

3) Fair: the student regularly attends internship with interest in the discipline: performs technical activities in known situations in a discontinuous but autonomous manner; in unknown situations performs the activities on the Tutor's indication without delving into the specifics.

4) Good: the student attends regularly and with interest: performs technical activities in known and unknown situations in an autonomous and continuous way with interest in deepening with the Tutor.

5) Excellent: the student attends regularly and exceeds the prescribed amount of internship hours; performs technical activities completely independently in both known and unknown situations. Deepens areas of disciplinary interest both with Tutor and in self-study with use of available resources (books, literature, case reports, comparison with other Tutors, Physicians, health care personnel, etc...)

Criteria for the composition of the final grade

The average of the scores from stage period corresponds to a starting score with which the student is presented, which will be integrated with the score of the theoretical practical examination
