Learning outcomes

The course aims to address the variability of disease trajectory, complexity of patient and family problems along a continuum of the following health problems: heart failure, IMA, angina, valvular diseases and arrhythmias The development of health problems in the stages of stability/ instability up to the advanced stage of
disease allows to: a) dwell on the relevance and need for assistance, educational and rehabilitation - with integrated and interdisciplinary approach -towards patient and family; b) reflect on the contribution to the care and care of experienced family patients; c) develop a territorial vision on resources and opportunities for home care; d) analyse the palliative and end-of-life approach, including for non-cancer patients; for symptom management and family support in difficult choices. Instability/exacerbation will be addressed with welfare protocol.
This teaching is based on the knowledge of the 1 year of "General Nursing and Applied Methodology" and "Pathophysiology applied to nursing" (evaluation of breath, meaning and signs/symptoms of hypoxia, dyspnea, principles of O2therapy; principles of caring and vigilance). The contents are linked to the modules of "Therapeutic Education" and "Community Nursing" for the trend of early discharge and need for support at the time of discharge. The teaching is also integrated with the Professional Laboratories provided for the 2 year course: making decisions for the management of the patient with heart failure in the stabilized phase and in the advanced phase. Teaching contributes to the following learning outcomes:
- ascertain and manage nursing care in patients with chronic and disability problems
- participate and collaborate in the management of diagnostic and therapeutic pathways
- monitor and monitor the clinical and psycho-social situation of patients by identifying signs of aggravation at an early stage
- identify and prevent exacerbation factors in chronic patients
- activate decision-making processes based on the patient’s condition, expected outcomes, the impact of the situation on the patient and his family
- support patient learning to self-manage health problems
